If this guy in the C18th replaced the whole of the interior design on a thin coin, how did he do it?

There’s a unique coin of Henry VII which appears to have the design of one coin and the legend (lettering) of another. Many people suspect it was altered by a guy in the eighteenth century – there is a suspect – who added the central design. But how did he do it? He couldn’t have used false dies, as these would have split the edge of the coin. How could he have used annealing without damaging the legend? One final point: the coin is about 10% light. Could what he did reworking the central design have caused it to lose weight?
